WAIVER, CONSENT, AND RELEASE OF LIABILITY

I, _________________________, assert that all documents, evaluations, and personal references (hereinafter referred to as "Credential File") be provided to employing officials who have legitimate need to examine them for the purpose of making employment/hiring decisions for existing vacancies in their organizations.

  1. I have requested that certain prior employers and other selected individuals submit to Northern State University Career Development & Placement Center their impressions, evaluations, and opinions regarding me and my abilities.  These individuals are advised that their comments concerning me are subject to my review.

  2. I have advised prospective employers that my credentials and references are on file with the Northern State University Career Development & Placement Center.  I hereby freely and voluntarily consent to full disclosure of my Credential File to any employer with bonafide interest in my qualifications as a new employee in their organization.

  3. I hereby waive and release any claim for liability I may have against any officer, employee or agent of Northern State University and the South Dakota Board of Regents System with respect to the assembling, maintaining, or disclosing personally identifiable information contained in my Credential File.  This Waiver, Consent, and Release of Liability shall be effective prospectively only and shall be effective after receipt thereof by the Northern State University Career Development & Placement Center.

  4. All files are the PROPERTY of the Northern State University Career Development & Placement Center.

  5. As of September 1, 1996, Credential Files will be stored for five (5) years from last date of usage.
